Label the Box, the Next Guy Could Be You
Thirty three annual revisions of one man's hazard log. Every row correct. Not one of them for the thing that finally sat him down on a basement step.
Behind the curtain+
Three stories on the front page were about the same thing without sharing a subject: a photographer who shot public benches for twenty one years and only discovered on review that he had been documenting hostile architecture, an engineer who ranked twenty one reasons for not becoming an EMT and found that nineteen were decoration, and an electrician thread whose governing maxim is to label the panel because the next person to service it could be you. All three are about a record outliving the intention behind it. The electrician piece and its comment thread supplied the actual hazards: the author's two broken heels, the taxonomy of electricians who cannot do each other's jobs, the attic in July. The remaining sources are pressure rather than content.
A hazard register is a slotted format and therefore one careless step from being a listicle in costume, so the rows were generated by a single invented life rather than by the day's stories. The two load-bearing sources, the bench archive and the ranked list of excuses, are deliberately given no rows at all. They are dissolved into the register's behavior instead: the scope field quietly shrinking across four amendments, and the trio of deferred rows that cite each other in a closed loop and have never once been assessed on their own merits. The unpriceable catastrophe and the uncappable exposure are folded into one row with a blank likelihood field. The whole piece holds the no-narrator rule absolutely, including inside the revision notes, so the biography has to leak through status changes and dates. The ending is the only thing that was fixed from the start.
PERSONAL HAZARD REGISTER

Origin: instruction of J. Mabry, journeyman. Verbatim: write down everything in this trade that can kill you. Mabry did not specify a stopping point.

Section A. Electrical
HAZ-001. Contact with an energized conductor. Likelihood: Probable. Consequence: Fatal. Controls (Rev. 1): Test before touch. Controls (Rev. 33): Test before touch. Test the tester. Test before touch again. Treat the panel schedule as a work of fiction. The breaker marked WATER HEATER is not the water heater. Note (Rev. 6): Near miss, 1999, Dunmore Street. No injury. No report filed. Row retained. Status: Open. Reviewed thirty three times. Never closed.
HAZ-002. Arc flash. Likelihood: Unlikely. Consequence: Fatal. Controls: Gear. Distance. Patience, which is not gear but is scored here anyway. Note (Rev. 17): Witnessed. Not the holder. Consequence rating confirmed by observation and not revised since.
HAZ-003. Client dissatisfaction. Likelihood (Rev. 1): Possible. Likelihood (Rev. 18): Certain. Consequence: Minor. Controls: Scope agreed in writing. Do not comment on the work of the previous electrician. Do not look at the work of the previous electrician for longer than is required to assess it. Note (Rev. 23): Downgrade requested by holder. Downgrade denied by holder.
HAZ-004. Being asked, at a social event, whether you could just take a quick look at something. Likelihood: Certain. Consequence: Negligible to Severe, depending on the something. Controls: None available. Status: Open. Permanently.
HAZ-007. Working on a system installed by somebody who labeled nothing. Likelihood: Probable. Consequence: Severe. Controls: Label everything. Label it for the next person. Assume the next person is unfamiliar with the building, in a hurry, and working from a drawing that is wrong. Note (Rev. 15): Drawings requested from building owner. Drawings confirmed to exist. Drawings not released. No mechanism identified for compelling a person to hand over a document about a thing you are standing inside. Note (Rev. 33): See HAZ-208.
Section B. Access, height, confined space
HAZ-012. Fall from ladder. Likelihood (Rev. 1): Unlikely. Likelihood (Rev. 5): Possible. Consequence: Severe. Note (Rev. 4): Incident, 1997. Both heels. Reassembled with graft. Eleven weeks, then four months of stairs taken one at a time. Note (Rev. 5): The likelihood revision was late by exactly one incident. Controls (Rev. 5): Three points of contact. Do not carry it up. Do not reach. Do not reach.
HAZ-013. Attic, July. Likelihood: Certain. Consequence: Moderate. Controls: Be out by nine. Water. Two men, so that one of them can go down and get help.
HAZ-019. Dog. Likelihood: Probable. Consequence: Minor. Controls: Ask. Then ask whether it is in the yard. Then look. Note (Rev. 9): Row retained after the holder began to like dogs. Liking is not a control.
Section C. Cumulative exposure
HAZ-031. Hearing. Likelihood: Certain. Consequence: Moderate. Controls: Plugs. Note (Rev. 14): Controls not applied, 1994 to 2007. Entry backdated. Note (Rev. 28): Status cannot be set to Closed. Status cannot be set to Open. Left as Ongoing.
HAZ-032. Knees. Likelihood: Not applicable. Consequence: Not applicable. Status: Not an incident. Note (Rev. 22): Reported by the holder’s wife. Entered under protest. Note (Rev. 26): No single event meets the threshold for entry as an incident. The threshold is the problem. Threshold not revised. Note (Rev. 30): Stairs timed. Result not recorded here.
HAZ-033. Solvent, lead, fiberglass, and the three other things nobody scored in the nineties. Likelihood: Unknown. Consequence: Unknown. Controls: Retrospective. Review procedure: the review consists of reading the row.
HAZ-035. Correction, delivered in front of others. Consequence (Rev. 1): Negligible. Consequence (Rev. 8): Moderate. Consequence (Rev. 20): Severe. Controls: None effective. Note (Rev. 20): This rating has moved in one direction only. No control has been identified in nineteen years of looking. The row is reviewed and the rating is raised.
HAZ-041. Accumulation. Likelihood: Certain. Consequence: Unknown until it is known. Controls: None scored. Note (Rev. 20): Entered after a conversation with a paramedic who worked a decade of highway and farm machinery calls without difficulty and then stopped, permanently, at a child with a scraped knee. The paramedic could not account for it. Note (Rev. 27): Reviewed. The paramedic’s register, if he kept one, did not have a row for it either.
Section D. Dependencies, and controls not owned by the holder
HAZ-061. Loss of partner. Likelihood: Unlikely. Consequence: Catastrophic. Controls: Home by six. Say the thing instead of planning to say it. Note (Rev. 11): Row created 14 May 2004, the day of the wedding, because the holder did not know where else to put it. Note (Rev. 25): First control assessed as effective. Second control not assessed.
HAZ-062. Dependency. S., b. 2006. Status (Rev. 13): Not a hazard. Logged in error. Retained. Note (Rev. 21): Dependency reclassified as mutual. Note (Rev. 32): Dependency reclassified as reversed.
HAZ-088. Sitting the industrial ticket. Status: Deferred. Controls: See HAZ-089.
HAZ-089. Cost of the industrial ticket. Status: Deferred. Controls: See HAZ-091.
HAZ-091. Timing of the industrial ticket. Status: Deferred. Controls: See HAZ-088. Note (Rev. 33): All three rows reviewed annually since 2009. All three deferred annually since 2009. No row in this group has ever been assessed on its own merits.
Section E. Unclassified
HAZ-200. Event of a scale that cannot be mitigated at the level of one person. Likelihood: [ ] Consequence: Catastrophic. Controls: None available to the holder. Exposure of this kind cannot be capped in advance by the party exposed, which the register notes without recommending anything. Note (Rev. 24): Likelihood field left blank. Opinion sought from four people who would know. Opinions received ranged from zero to certain and were in every case delivered with total confidence. Note (Rev. 33): Reviewed. Field remains blank. Row retained. A register with no blank fields has been completed by somebody who was not paying attention.
HAZ-201. Being wrong in a way that does not become visible for twenty years. Likelihood: Unknown. Consequence: Unknown. Controls: Label the box. Status: Open.
HAZ-207. Cessation of work. Likelihood: Certain. Timing: estimated 2029. Estimate revised 2014, 2019, 2023, each revision later than the one before it. Consequence: Unscored. The register has no consequence scale for this. Controls: Pension. Second pension. Nephew. Note (Rev. 30): Parts are observed to outlast their rated service life by decades, including parts in people, including parts moved from one person into another and still running at a hundred years old. The actuarial figure is a floor and not a ceiling. Note withdrawn at Rev. 31 as out of scope. Note restored at Rev. 32.
Revision 33. 2 March 2026.
Rows reviewed: 207. Rows closed: 0. Rows opened: 1.

Date of event: 2 March 2026. Verification: address checked against invoice dated 11 August 1998. Invoice signed. Work described as a service upgrade and the labeling of thirty one circuits. Handwriting consistent with HAZ-012, Rev. 5, written the same winter. The holder has no recollection of the work, the house, or the customer. Consequence: the holder sat down on the basement step for a period estimated at twenty minutes. No injury. No damage to property. No report filed. Controls: Label the box. See HAZ-007. See HAZ-201. Status: Open. Likelihood: Not assessed.
